instead of seeing the lines "take the skin and peel it back / now doesn't that make you feel better?" as meaning that he is peeling back the skin of the lined-up "pigs", i think he is peeling back his *own* skin for their benefit or pleasure. the character has a marked tendency to abuse himself or let others abuse him. i think he is making a point about how society wants to use people, smash them and discard them for its own few moments of gratification. "the pigs have won tonight / now they can all sleep soundly / & everything is all right" means that they've taken enough of him, they're sated *for now*, and they can go back home with their bellies full of the experience, until the next time they need to "bite chew suck away the tender parts" of some other unfortunate victim...

Maybe you will say that i'm a sex addict, but it is not the case. I think trent is describing an orgy or something like that. he commands a boy or a girl. he apparently wants him or her to suck his dick by first peeling the skin back, watching it come down and he said that he doesn't like the taste, the look and the smell of it (sperm). Although it could be disgusting, the narrator ask if it doesn't feel better anyway. The stains like the blood on your teeth could be sperm too. there's also few words relating to the mouth. For the pigs, i think the narrator sees his her companion as a sucker, an idiot, a pig.
